Left Heart Procedure

The Left Heart procedure is used to gain information on the Left Ventricle and Aorta to asses the function of the heart. This procedure also includes taking imagery of the Right and Left Coronary Artery’s to determine if a blockage is present as well as the extend of blockages

Supplies

  • Cath Pack Tray
  • 6Fr Sheath
  • JR 4 Catheter
  • JL 3.5 Catheter
  • Exchange wire
  • Baby J Wire
  • TR Radial Compression Band
  • Radial Cocktail – Verapamil and Heparin

Procedural Steps

  1. Prep the patient with a sterilized solution for the areas you will need to access.
  2. Drape the patient with the drape that comes in the Cath Pack Tray. Leave openings for the areas you will need to access.
  3. Locate the Radial Artery and insert your 6Fr Sheath.
  4. Inject your Radial cocktail to prevent vasospasm.
  5. Insert the Baby J Wire into the JR 4 Catheter, insert the catheter into the sheath. Advance the wire into the ascending Aorta until you can see the wire folding on the Aortic Valve.
  6. Cross the Aortic Valve with the Baby J Wire, pass the catheter into the Left Ventricle and remove the wire. Connect the back of the catheter to the ACIST Machine to record pressures.
  7. Record the Left Ventricle pressures and pull back to cross the valve to record the Aortic pressures.
  8. Position the JR 4 Catheter into the Right Coronary Artery. When ready record the images with CIN imagine and inject contrast into the artery to assess blockages.
  9. Disconnect the ACIST Machine and insert Exchange wire into the JR 4 Catheter.
  10. Advance wire as far as you can then removed the catheter over the wire leaving the wire in place.
  11. Insert the JL 3.5 Catheter and remove Exchange Wire.
  12. Connect the ACIST Machine again and locate the Left Coronary Artery.
  13. When ready record the images with CIN imagine and inject contrast into the artery to assess blockages.
  14. Disconnect the ACIST Machine and advance Exchange wire as far as you. can and remove both JL 3.5 Catheter and Exchange Wire together.
  15. Flush 6Fr Sheath with Heparin to remove clots.
  16. Prepare TR Band over the sheath and inflate with 15cc Air.
  17. Remove the sheath and slowly reduce amount of air until flash of blood, inflate 2cc air above the flash point to stop bleeding while preventing artery occlusion.
